Birth name
2) In the future I would like to file an application to change my birth name however before I do, Is it legal to fill a ''DBA'' under my professional name ''stage name'' in Maryland & New York, Nevada?

In NY, the answer is yes. In the other states, I suspect it would be the same, but I cannot give you a definitive answer. Do some research on each state's website...
