Legal Question in Business Law in Nevada

I co-own a yoga studio in Nevada. Even though we have studio liability insurance, we require every person to sign a waiver when they first visit our studio, prior to attending any classes. Some attendees, particularly out of towners, will only provide their first and last name on the waiver and sign the waiver, but will refuse to provide other info requested on the form such as their home address and phone number, or the name and phone number of an emergency contact. In the event of an injury on our premises we feel this information is essential. Can we prohibit someone from attending our classes if they refuse to provide this information? Can we require that they present us with I.D. (such as a drivers license) to confirm that the home address they provide on the waiver is true and accurate? Thank you.

As a Franchise Attorney I can say this. You need to show your waiver etc. to a local attorney. Waivers generally don't work if there's negligence involved. Consult with a good business or franchise attorney in your area for specific advice.
