How do you file a Bill of Costs after a default judgment

1) How do you file a Bill of Costs after the Court has entered a default judgment, I can't find the rule on Bill of Costs? and,

2) Do you file the Bill of Costs with the default judgment, or with the notice of entry of judgment, or after the 10 days after notice of entry of judgement has expired? and,

3)Do you have to do a Memorandum of Costs, and if so, where do we find the form or where is the law explaining how to do one?
