Name Change and Disclosure
I am relocating to Las Vegas and I noticed that in order to obtain a work card, I have to subject myself to a background check. On the form, it asks if I have any 'Alias or Maiden Names.' About 18 years ago, I legally changed my name. Do I have to disclose my previous name? It wasn't due to marriage, more personal/not wanting to be associated with my family's name.

The question is trying to find out if you have ever used another name so that, when a background check is run, they include your former name. I would disclose it as a former name. Otherwise, you run the risk of being fired when the company finds out you failed to disclose it.
