Adding another last name to minor childrens current name
I am divorced and reside in Nevada with my two children. My children and I have my ex husbands last name. I am in a long term relationship in which we have chosen not to get married but would like to add my boyfriends last name to mine and my childrens by hyphenating. I need info on how to do concerning my children so it is legally accepted for such things as school registration and ID. As far as I know I do not have to get their biological fathers consent in order to hyphenate their name by adding my boyfriends. I do not know what legal forms to get or how to go about this.

You cannot unilaterally change your children's names. You must get their father's consent and Petition For Name Change. If the father does not consent then you must serve him with the Petition and put the matter on for a hearing before the Judge.

If you chose for your children to retain their father's name at the time of divorce, you will be unable to do so on your own in Court. You will have to serve formal notice of the Petition for Name Change on your ex-husband to give him a chance to respond in Court.
If your husband were to fail to respond or show up in Court, you probably could receive a default judgment.
