Im having a child in 5 weeks. My fiance's mother has a alcohol problem and I do not want her around my child. I need to find out if there is a legal document or can I type a letter and have the father sign it saying that she is not allowed to be around our daughter till a certain age with supervision.

She has no rights to see your daughter so if you can get the father not allowing his mother around your child, a court would enforce it. You would need to put a penalty if he violates the agreement.
The problem is you may need to go to court once it is violated and that could wreck your relationship with your fiance.
