Child's Name Change

My daughter began using her stepfather's last name at the age of 15. When she started 10th grade at a new school, she used his last name instead of her given name. When she took drivers ed her learners permit was in her new name. She is now 34 and has gone by this name ever since. She has her license and Social security card with her ''new'' name. Should she expect any problems obtaining a passport?

Post 9-11 restrictions got much tighter on this type of situation. I would suspect that she will have problems if applying for a new passport because her birth certificate won't have the correct name. If it's a renewal passport, perhaps she won't have problems.

If she wants to have her name legally changed (which gives Vital Statistics the ability to issue an amended birth certificate) then she can contact our office. She should do a legal name change. Passport officials will want to see her birth certificate and if there is no Order for a name change will issue the passport only in her original name.
