legal way to apoint my mom temporary custody of 2 kids

the father and i are having seperate (not w/each other),legal issues which could put us in jail. We have 2 daughters,dobs are 6/27/02&4/2/02 we've lived w/my mom for 4yrs almost and need to have in place the proper forms apointing my mom as legal temporary gardian or permanant if him/i are no longer capable of reclaiming our rights back.ex.death,permant brain damage etc. Please advise and thankyou for all the help

You can appoint mom as temporary guardian for up to 5 months. See form on findforms.com. Or you can have your mom petition family court to become guardian and you parents can sign consent forms to this being done.
