I need to know what form do I need to add my wife to title on a las vegas nevada property.

Quit claim deed or grant bargain and sale deed. I can prepare if you need it prepared.

The simplest way is for you to give her a quitclaim deed from yourself to her and yourself as joint tenants. Then if one of you dies, the other owns the property.
However, it the property was your separate property prior to the marriage and you quit claim it to yourself and her you are making a gift of separate property to the marriage community and if you ever get a divorce, she may well own half the property.
