Declaration and Writ of Summons

Could you please advise me on how to write a Declaration and Writ of Summons. I have an ex-tenant that owes me a lot of money that I need to sure for. I consulated a lawyer but he said it would be about $5,000 for his services so I am going to have to file this case myself and hopefully have a positive outcome. I picked up the Writ at the courhouse today but do not know if there is a certain outline to follow. Could you also recommed a good book or website to consult to read up on this matter. Any help is greatly appreciated.

Ask another lawyer for his estimate of costs. If it is under $5000 it can be a small claims matter and a lawyer can do that for a lot less than $5000.
