Transfer of property deed
I hold a mortgage in my name. Can I transfer the deed into the name of my two children to avoid probate proceedings and taxes

I would seriously suggest you meet with an attorney to review your situation. Transgerring to your children may have significant tax impact. Also, if one of your children should be sued, you risk the loss of your home.
In many cases in New Hampshire, probate is a very simple process and there may be no additional taxes.
