BC/BS PPO refused to pay for referred services

Plan book from former work stipulates 100% coverage for Physical therapy with no-co pay on one page, with co-pay on another.

Conflicting information. I requested therapist call regarding co-pay before beginning therapy.

Nine visits to therapist no co-pay charged at that time.

BC/BS PPO sends statements, refusing to pay about $150.00

In therapy charges. $90.00 in copay charges added.

I resubmit claim form then forget about it.

Three years later law suit from collection agency.

Went to court. Judge demanded I sue BC/BS and he would

Hear the case again in January jointly.

The total bill is under $300.00 including court filing fees.

I don�t want 7 years of bad credit for this, nor loose my job for time out.

Collection agency refuses to negotiate. Can I file a motion to Dismiss? Am I obligated to sue BC/BS?

How do I do this?
