What is the proper procedure for someone to put a levy against your bank account

My daughter went to a pre-school in 1991 when she was 2 yrs old from Jan 91-May91. I received a copy of my credit report 3wks ago & it had a judgment against me as of 4/2004, but the filed date was blank from this pre-school. On 1/20/05 they put a levy against my personal checking account for 1,100 & 100 from an account I share with my husband with no prior contact. My daughter will be 17yrs old May of this year. How is this possible from something that happened over 15yrs ago?

You have to go the Court where they got the judment and see when and how they served the papers. That is the only way to get a judgment. Also, the notice you got should tell you what to do to get a hearing, by writing to the Court with a copy to the attorney. If you want an attorney to help you, it should cost about $1,500 as a retainer. You do not want this on your credit history.
