The dentist
This dentist refer me to specialist and with the referral, else a fax saying that I was a drug addict which I'm not Is this defamation and lack of professionalism Can I sue the dentist?

It maybe false but I doubt it would constitute actionable defamation. Even if it did, attorneys do not take defamation cases on contingency fees. They take them on retainers, which means you pay up front.
