Acknowledgment of Servment

I am filing my divorce myself. The paper work is not a problem. I need to serve my ex husband legally, and I was told I need an Acknowlegment of Servement form, but know one can tell me where to get this form of what it looks like. Please tell me where I can find this form, or possible give me a website from where I can down load this form.
