Changing the name of a minor

I am divorced. I am now with my fiancee and have 2 children from him and one from a previous marriage. My oldest daughter wants her last name to be the same as hers. I do not know if the father would relinquish rights-but he has not been around. She receives disabilty payments from the state because he is disabled. I have full custody. How would I go about changing her name? Do I have to wait unitl I am married to change her name to my fiancees last name so it matches her other sisters? Also, do you know the approximate cost of doing this?
