Child custody

I have custody of my grandson mother or father dont visit or are involved for 3 years can i get perm custody or guardianship of the baby. Mother took me to court once she was told to visit 1 or 2 times per week for 6 weeks saw the baby 1 time for 30 min never came back moved to miami dont know were she lives down there what can i do
