Legal Question in Family Law in New Jersey

my daughters father and i are seperated. he has never been an active role in her life, he doesnt help financially, has a VERY extensive history of voilence, drug& alcohol abuse.has been incarserated numerouse times & this last arrest was for a high speed care chase & assulting law inforcement officers.a few months ago he was given supervised visitation by the courts,during the 3month visitation period he only showed up 2 times and the report says he was un attentive to her needs, was quiet but distracted & at times seemed fustreted. we have a long history of domestic voilence & this last incident he sent me death threats via email . i have the prented evidence.My daughter was born with digestion problems & i have tried many times to get her father involved with her life by inviting him to dr's appointments and health related events but he never shows. can i get sole legal & physical custody to ensure desicions made concerning her helth issuses are in her best interest

It would seem that you do not need sole custody because the father has no interest in her medical care.
