Divorce filed in 1988

The divorce decree does not state that I,--name removed---name removed--will return to my maiden name. What process do I need to take to get an amendment to state this? Divorce papers filed in Camden NJ.

You can file a motion to amend the final judgment of divorce to include the name change which you desire.
