Legal Question in Family Law in New Jersey

I had been divorced for four years. My ex who provided medical insurance for children never give me any info or insurance card. Children had been covered under mine. Can I ask the court to order him to pay half of my premium? Although, we had joint custody, but my ex never show up on the visitation day nor exercise his parenting rights, can I filed for sole custody? And he never provided me with his life insurance as PSA and I supposed to get half of his pension plan through his employer, how can I get this?
