Legal Question in Family Law in New Jersey

there are thousands of cases,where DYFS puts children in homes someone has proven to be dangerous or fatal,and then something happens to that child,in that home?this agency who is supposed to protect children,is famous for this.why aren't they being prosecuted for endangering,and failure to protect?(especially where there is proof of violence,drugs,and other crimes)
