Legal Question in Family Law in New Jersey

My ex-husband has never paid unreimbursed medical expenses for our 2 children. We have been divorced 10 years and I have been asking for him to contribute ever since although I only have a couple of receipts for letters mailed to him. It was mostly phone calls. He has threatened every time to quit his job and run off so he wouldn't have to pay cs either. I believed him at those times because he has a history of alcoholism and dui's that he fled CA from and I really needed the cs payments. He has only seen the kids about 10-15 times during the last ten years and I have had to bring them to NJ from MD to see him. He has never initiated. He claims that he has not approved any bills but out psa does not state that he has to. It only states that I must consult with him which I did but he would never agree. These bills include ortho, psychiatrist and psychologist. The judge ordered him to pay 25K but he got a lawyer and got it appealed so now it's pending a plenary hearing. I can't afford a lawyer and have been doing this on my own. The judge has let me do phone hearings with her but I will have to go to NJ next time for the plenary hearing. What do I need to do to win this case so he will have o begin paying his share of these bills? Pleas help.....
