Filing a substitution of attorney

I currently have a post judgement motion before the Family Court. The matter has already been heard in October but there were a few things that were left open ended. At the time of the hearing I had an attorney representing me. I just terminated my relationship with her last week and just received a signed Substitution of Attorney form this week because I have to finish this matter Pro Se. My question is regarding the filing of the form; do I need to go to the court house directly to file this or can it be faxed or mailed to the judge's chambers? Also, do I send just the form or do I need to send an additional letter to the judge stating why I terminated the relationship and why I will be representing myself. Finally, what do I need to send to the opposing attorney to let them know that I will be representing myself from here on out?

Re: Filing a substitution of attorney

Find the address for the Family Court Clerk for the County this is filed in. Mail the Court Clerk the original and 1 copy asking them to return the copy marked "filed." Send copies by cc to the Judge and the opposing attorney at the same time.

There is no need to give reasons unless the Judge requires that information as a condition for approving the substitution.
