Judgement of Divorce

My judgment states that no parent shall burden any out of pocket medical expenses for said children. However, I was told that the child support guidlines are 250/per child per calendar year. Which holds true, the judgment of divorce or the child support guidelines?

How can the children have no medical coverage? How will they pay for it?
