how to keep nursing home from taking all of inheritance

My mother is 82. She has many assets worth a good deal of money. If she should be put in a nursing home where bills are quite high is there legal ways for her to give her money and property to her children without any legal problems. I understand a person can give money as gifts to family members once a year that is none taxable and can she claim such gifts on her income tax. Would it be wise to change the name her house is in to her childrens name?
