obtaining supervised visitation
my step sons biological mother is causing him emotional grief, he is not allowed to call me mom or anything resembling mom. she tells him he can tell the judge he wants to live with her, she is making him choose between myself and my husband (his father) and her. he's 11 and she lets him watch R rated and unrated very inappropriate movies such as the latest one Death Proof by Quentin Teratino about sex, drugs and explicit violence, when he speaks to her on the phone he hides in the bathroom or his play room. she tells him about her child support we ''make her pay.'' she tells him his father never came home from work when he was little, they separated when he was 4 - she abandoned them to become a rock star. she makes him call her boyfriend (who is married to someone else) Poppy. These are only half the issue I just want to know how to go about getting supervised visitation.

The simple answer to this is that you would need to file a Motion with the Court to establish supervised visitation is in the best interest of the child. However, the proofs and costs of same is often prohibitive. You really need to speak with a Family Law Attorney to review the options.
One comment: it is not a good idea to have the child call you "mom" or anything related to that term. It is emotionally difficult for most children and really angers the bio mother. Same for "dad" and the bio father. I realize that this is only one small issue in your entire situation. But I thought it was worth a comment.
