relinquishing rights

Ex-husband has not seen children in 7 months, has not talked to them for 4 until yesterday. He spoke to 12 year old daughter and told her he was just in hospital for going crazy-he has a history of many suicide attempts. Children do not want to go out to Ohio where he lives to see him and stay there-they would like their visits to be only in NJ-they and I fear for their lives. He is dangerous to himself, as well as others. What can we do to protect my children?
