I am seeking a temporary separation from my husband are there some kind of forms I need to file?

There are far too many issues in your question to be able to answer it properly. And there is no "form" that could be completed to assure that all issues are covered.
For instance, if the two of you own a house together, who stays and who goes? Who pays the bills? What happens if that person doesn't pay? Are there kids? If so, who do they stay with primarily? What is the parenting time scheudule? And how "temporary" is this? Are you going to try to get back together? If so, what are the two of you doing to make that happen?
You need legal advice so that there is a complete understanding of what happens in the interim and in the long term. Keep in mind that this advice is given based on the little bit of information that you have provided in your question. My advice may change based on other information. If you are only separating on a temporary basis, there is no requirement that you file papers with the court. If you are seeking support during the separation you should speak with an attorney.
