How do i get sole custody of my child?

I have a one month baby, his father through my 9 months pregancy has verbally abused me, telling that if i put him in child support he will take my child, others thing he has said was he going to take the baby for the weekend and he will bring him whenever he want and also take him. He wasnt there for me during my pregancy i dont receive a cent from him. My father insurance paid every visit to the doctor i had and also paid for the hospital bills while i was in labor. I fear for my life and my child . The father of the baby did sign the birth cert. But legally im the only one who provides for the baby. He wants to take the baby and i dont know where he lives he uses 3 different address two different date of birth ( years). Could i get sole custody of the baby and limit his visitation rights... Do i have a chance of winning this case. Please help me i dont know what to do im scared that he will take my child away from me and i'll never see him.

Such a young child should not leave your house. Allow the father to visit the child at your house. He can not take custody from you so file a complaint for custody and child support immediately.
