Sole custody and relocating
I have sole legal and physical custody. I have a restraining order. He was in jail for violation of that. While in jail he signed a consent order suspending his visitation until further order of the court. He has been out for about a month. Recently he has started calling and harrassing me again. I want to move out of state. I know I have to get court's ok, but does he have to know where I am moving? I worry that he'll move there as well. We have been divorced for a year and a half, but separated for over 3 years and he still threatens any guy I talk to and threatens to kill me and any other guy if I get involved with someone. Does he have to know where I am going?

He seems to pose a substantial threat to you even now. You should report the recent threats to the police so that he can go back to jail where he belongs.

You do need to file a relocation motion with the court. As part of the the Domestic Violenence Act, your address can remain confidential. I would also report all the violations to the police because he will be re-arrested.
