terminating guardianship of minor

i am looking for legal forms to terminate guardianship, both parties agree to terminate guardianship we just need to fill out papers, stating why & sign & notorize paper and bring to deputy surrogate in new jersey, can someone please tell me where i can get these forms

Your question is unclear.

If this is a guardianship for a child, and if the child is still a child, then someone needs to care for and raise that child.

If this is a guardianship for someone who was ill or disabled, then yes, you can terminate the guardianship if the person does not need a guardian's help any longer.
