Unreimbursed health related expenses standards

What are the standards related to child support, for the payment of unreinbused health related cost for children. Focusing on permissible type products and services for submission for payment by the required obligor?

The only standard is that the medical services received were reasonable and necessary and that the expenses incurred were reasonable and ordinary.Where there is no court order the parents have an equal obligation to pay for such medical services to the extent that they are financially able. The courts normally assign individual responsiblility to individual parents based upon each parents percentage of the total joint income.

These questions seem to be a bit overly broad. If there is an actual situation it must be explained to me in a more precise and accurate fashion and in much more detail.
