You have a fight with your child's mother. It will likely end up in Court unless you and she can reach an agreement.
You really need to sit down with an attorney and get some guidance.

Regardless of that, you need written consent of the mother to take your daughter out of the country. Hopefully, you have time to apply to the Court for getting this resolved before your planned trip begins.

No, but you'll need a motion to get it from her if you want it. Doubt you need an attorney for this one, but it never hurts to consult with one.
