Information Subpoena and Written Questions

I got the subject document recently. The last paragraph states '' Be sure to sign and date your answers and return them in the prepaid return envelop within 14 days from the date you receive this subpoena''. There was not such a envelop in the package and so I don't know who to sent it back to, in addition the cover page of the subpoena is singned by Plaintiff only but not by the court's clerk.

If I don't know who to send it to, and the document is not signed by the court's clerk, is there a problem and if so, how do I deal with it?

The interrogatories should have a caption that gives the name of plaintiff's attorney. That is the return address. If not, contact the clerk of the court.
