i am seperated, both our names are on the house, how do i remove myself from the title?

The literal answer is you would give something called a Quitclaim Deed to your spouse in order to relinquish any ownership or marital rights to the house. You should note that this does nothing to eliminate your liability for the mortgage loan, if any. The larger question, however, is why you would want to give up your interest in the house at this stage. You are separated. Do you have an attorney? Get divorced. There is almost no reason not to.
