Reapplying H1B

HI

I was working in India and have a H1B which is valid till dec 2006, in which i never travelled.Due to personal reasons i had to travel to US in h4 which inturn cancelled my h1b without prejudice.Now I intend to work here, Do I need to freshly apply for a H1B? Even if I have to, do i fall in the cap?
