I renewed my green card and when it came my name was spelt incorrectly. How do I fix this?

In order to request a corrected card, you must complete and submit Form I-90. First, you have to figure out whether you (or the person who helped you fill out your application for permanent resident status) made the mistake, or whether USCIS made the mistake. If you review your application for permanent resident status and find that your name is misspelled on the application or the identity documents you submitted with it, then it's probably your fault and you have to pay the Form I-90 filing fee. If the information you provided was correct, then USCIS is at fault and you don't have to pay the Form I-90 filing fee. You must send your original card along with Form I-90 to obtain a corrected card based on its error. In addition, the instructions request that you submit proof of your correct name and biographical data even if this information was previously submitted with your application for permanent resident status. You may want to explain in a cover letter that you provided the correct information to USCIS with your application for permanent resident status and that you are including a copy of the application and supporting documents you previously submitted as evidence that the mistake was theirs.
