Legal Question in Intellectual Property in New Jersey

I've been the victim of Identity fraud twice; I'm frustrated with having to write-down and/or give my social security number out to people/organizations that required this for some form of service or process; but take absolutely no measures to secure, encrypt and/ or mask the information from prying eyes. I need an NDA type of form that I can have the recipient of my information sign before I pass on to them my personal information. As far as I'm concern; if you want the information, you have to make yourself responsible for this, and liable for the loss of it. Maybe this will have companies think twice before asking for information.

An attorney can draft it for you. However, a better solution is to request a copy of the company's privacy policy before you give out personal information. Anyone who collects personal information, such as your social security number, must have a privacy policy and security system in place to protect the data.
