Your question regarding the details of an affidavit of merit, is a procedural question, which ordinarily would be handled by an attorney in the course of pursuing a lawsuit.

Your question implies that you are considering the pursuit of a malpractice suit without an attorney. As you move forward, you will need to decide, whether it is worth paying an attorney for advice on this, and many other issues you will encounter.
