Adding a person's name to an existing deed

I am currently paying a mortgage on my house. I want to know if a person can be added to the mortgage/deed (not quite sure where it needs to be added so it will also be considered their house). Also, how would this be done? Would I have to re-finance to get their name on the papers? Can a lawyer do it without re-financing...just completing some paperwork? Is there some paperwork that I can fill out without having to go to a lawyer or re-finance? I have no idea what to do and would just like some basic information as to the best way to add this person's name to the paperwork. I would really appreciate your help.

If your sole objective is to add someone to the Deed, a new Deed is all that is needed. You need to stay on the Deed or it would be considered a sale and accellerate the mortgae. The person added does not become responsible for the mortgage without signing a new note.
