What form would be used to correct a mortgage assignment in NJ?

The mortgage company lawyer assigned the mortgage to himself from MERS in October, 09, before filing a civil foreclosure suit. I had already filed RPOA and full conveyance of mortgage in 5/09 and filed copies with county registrar. I need to locate the appropriate form to file of notice of correction, i.e. affidavit or declaration to correct title record (with copies of original filing) and then assign mortgage to a trust.
