In NJ, am I required to add my spouse to the home title if I add her to the mortgage in a refinance?

That's a question the lender will address. In any loan secured by a mortgage on real estate, no matter who the borrower is [the only one who will sign the note], all owners of the property have to sign the mortgage.
You can't just "add" someone to a mortgage. It's the same whether it is a purchase money mortgage or a refinance.
