Refinance to get out of chapter 13

I had refinanced in order to resolve my chapter 13 filing. The representative of the lending firm who handled all documentation falsified a payoff letter. The State took the company to court and they were found quilty and fined 5k. The employee was fired but I still had unresolved closing costs that needed to be addressed. After 10 months of being transfered from one person to another in this firm, they sent me to their legal department in order for them to address my unresolved issues. Their attorney sent me a letter stating all closing costs were correct and they had nothing more to say or would investigate any further. I had paid Home Insurance twice, (have my cancelled check) my second mortage was paid in full but the amount the lending firm sent was quite a bit different than on my closing fee statement (HIPPA - unsure of acronym). I have the payment report from the banking institution of my second mortgage. All in all, I'm out approximately 30k and very short on money. What are my options?

You need to hire an attorney and take action. I do not think any free advice will help get your money back.

You might, I am not sure, have a consumer fraud claim against the company which provided the false and defrauding documents. If you do, then you can sue for three times the amount you lost plus attorneys' fees.
