I'm refinancing my primary residence and would like to add my adult children to the title in order to avoid probate issues later on. What is the best way for us to hold title?
Also, this is "affordable housing." Am I allowed to add family to the ownership of the property?

There are no real probate issues. Some states have expensive and difficult probate; NJ is not such a state. Avoiding probate here should not be what determines ownership of your home.
Affordable housing has it's own requirements, so you'd better check on whether anyone else is permitted to be in title with you or if that would disqualify you.
The best way to assure who gets what after you die is to have a will, and to keep it up to date as circumstances [yours, or your family's] change.
