Blank form for Spousal Disclaimer

I would like to see a sample blank form used for a Spousal Disclaimer for funding a credit shelter trust addressed to the Executor of the Will.

Preparing a Qualified Disclaimer is not something you should try to prepare on your own with "a simple form."

You are talking about serious tax issues and possibly big tax dollars involved (both federal and NJ estate taxes may apply). You really should see a qualified estate attorney to assist you with this.
