I have a child with cancer and am having a benefit I applied for a fed id # and I am trying to open a bank account but they will not let me until I have a trust drawn or fill out a trust form. I need help to find this exact for in order to open the account.

There is no "form" for setting up a trust. Your situation really requires that you consult with a lawyer who has experience in setting up special needs or supplemental needs trust and in dealing with medicaid planning. Look for a lawyer in your county or nearby who does.

I agree with Bob, but have some confusion as to exactly what you are trying to accomplish; set up a trust to cover the child's expenses, raise funds for research and/or to help other children, or something else. More information is needed for a better response.
