Is there a form the Executrix must sign prior to taking the 5% fee or is the final accounting document suffice for the accounting of said monies?

As Executor and/or attorney for an estate, I traditionally provide all estate beneficiaries with an informal accounting and what I term as a Waiver Consent and Approval form to sign. The form acts as their consent to all I have done and approves the payment of all bills and fees, including legal and executor fees. I suggest getting something in advance of your taking any fees, to avoid any problems.
