hello, looking for a format on a form to write on requesting a copy of a will

There is no special form or letter. You do not say if the Will you are seeking is of someone alive or deceased. If alive, you need permission of the maker to get a copy. If deceased, you can go to the Surrogate's office where it was probated and see it, or for a small fee, get a copy.
