what kind of form can I get to represent my sibling in a Estate benefit claim?

I would suggest a Limited Power of Attorney giving you the authority to initiate, pursue and litigate and/or settle the claim in the name of and on behalf of your sibling, should be sufficient. It should also authorize you to retain counsel, execute retainers and negotiate fees, sign any documents, etc., and spell out how any recoveries should be handled.
