Trust fund agreement
I need to set up a trust fund for my son where he receive a stated amount of money each month.
What kind of trust is this?
And where could i downloand a form like this?

There is no simple do-it-yourself trust form. You really should seek counsel of an attorney experienced in setting up trusts. You may be taking a risk if you are older and may need nursing home care in the future, and if your son has special needs, you would make him ineligible for important benefits if you make trust funds available directly to him.
